Primary care is among the most important contributors to health, improving nearly every measure of wellbeing by providing a usual source of care, early detection and treatment of disease, chronic disease management, and preventive care. In an effort to improve primary care delivery in California, health plans are working together on the California Advanced Primary Care Initiative. This initiative aims to strengthen primary care by creating a statewide model that emphasizes comprehensive, whole-person primary care.
