10 Building Blocks of High Performing Primary Care Series

In 2010-2011, the UCSF Center for Excellence in Primary Care (CEPC), an organization that identifies, develops, tests and disseminates innovations to improve primary care, conducted an intensive study on existing high performing practices and clinics to identify similarities in how they deliver primary care. These practices varied in size, resources, staffing and the populations they served. Yet these practices had common characteristics in which they provided high quality, accessible and person-centered care. The fundamental elements that were identified among them led CEPC to create the 10 Building Block of High-Performing Primary Care. The 10 Building Blocks focus on design elements that are largely within the realm of control of the practice and the provider organization and thus can function as a roadmap to help practices to transform.

CEPC partnered with the California Quality Collaborative (CQC) to develop a series of 15-30 minutes on-demand webinars that can help practice coaches, doctors, nurses, medical assistants and anyone who is interested in the healthcare system to understand foundational practice transformation principles within the 10 Building Blocks of High-Performing Primary Care framework. These webinars use a tactical approach to address core elements that facilitate success in the following priority areas for the CQC’s Practice Transformation Initiative:

  • Building Block 3: Empanelment
  • Building Block 5: Patient-Team Partnership
  • Building Block 6: Population Management
  • Building Block 8: Prompt Access to Care

Overview and History
The first webinar in the UCSF CEPC and CQC series presents an overview of the 10 Building Blocks of High Performing Primary Care, detailing its development and how the framework addresses the quadruple aim to improve health, patient experience, and the work setting for healthcare providers and staff while making costs more affordable. Building Block 3 – Calculating and Weighting Patient Panels
In the second UCSF CEPC and CQC webinar, we dive into empanelment, Building Block 3, with a how-to guide for creating the ideal patient panel by identifying active patients, determining panel size, and calculating supply and demand. The third UCSF CEPC and CQC webinar of this series continues to explore empanelment, Building Block 3, in a practice: operationalizing and managing patient panel sizes by keeping panels updated, balanced, and stratified based on the patients’ needs. The fourth webinar in the UCSF CEPC and CQC series highlights the Patient-Team Partnership, Building Block 5, and demonstrates how health coaching strategies can be used to engage patients in care and leverage the expertise from both patients and care teams. The fifth webinar in the UCSF CEPC and CQC series is the first of two webinars about population management, Block 6, and introduces the importance of panel management by using tools such as a patient population registry, an EHR health maintenance screen, clinical practical guidelines, standing orders, and trained panel managers. The sixth UCSF CEPC and CQC webinar is the second of the two webinars that expands on Population Management, Block 6, with the six core steps to caring for patients with complex needs. These steps support care teams to assist patients and caregivers, manage medical conditions more efficiently, consider the context of patients’ lives, improve health and well-being, and reduce the usage of expensive medical services that can be avoided.
